No, we replaced the menu bar with the "opera menu" a few versions back.
Of course, the layout of the menu entries in the opera menu is quite
different from what it is in the menu bar.  If you really want the menu
bar back, you can reenable the menu bar by choosing "show menu bar" in
the opera menu.

(Mostly, I think the changes are for the better, but some of the changes
definitely breaks with "standard" behaviour.  Which is a bad thing.)

I'm guessing this is part of the window decorations, so it is really
part of the window manager rather than opera.

That's probably the "main bar".  I don't think it is turned on by
default, though.  (You can turn it off from "opera menu"->Toolbars->
main bar).

That's the "opera menu", the current main menu of opera.

There should also be a waste basket at the far right on the tab bar, as
well as a plus sign just to the right of the last tab.

That sounds like it is probably the address bar.  By default it should
have "back" (left arrow), "forward" (right arrow), "reload/stop"
(circled arrow/x), "Log in" aka "wand" (key), address field (text entry
widget with url type indicator in the right part and possibly some icons
in the left part), search field (drop down widget with search provider's
icon in the right part and search button (magnifying glass) in the left
part).

Those are links given on the page you're looking at.  It should only be
displayed if the page actually have defined some of those links.

You should also have a "status bar" at the bottom of the window with
"panels", "link", "unite", "turbo", a status area, a small view menu
(small up arrow) and a zoom slider.

It's a mess, but there shouldn't be much that is repeated.  Arguably,
the mess isn't much worse than it has to be while still giving
reasonably easy access to the most important features of the
application.

You may want to try running opera with the "personaldir" command line
argument to see what opera looks like when there are no old user
settings around. e.g.:

opera --personaldir to_be_deleted

where "to_be_deleted" is the path to where opera should read and write
its user settings.  It is typically a good idea to use a non-existing
path, so you can safely delete it afterwards.
