Looking For Flat Rate Shipping Based on Items Ordered

I can't seem to find this in the shipping contributions section (maybe its under a different name), but I'm looking for a module that will hard code a shipping price based on quantity ordered.

Ex:

1-5 - $5
6-10 - $9
11+ - $16

something like that
