Attributes/Price in text boxes?

I see the code just posted for converting "add to cart" to a text box allowing the customer to enter how much they want of a given item.  Any ideas on how I could extend it to do something like this?

COLOR:    Red      Blue      Green
SIZE:
Small        ___        ____      -_____
Medium     ___       _____      ______
Large       ____      _____      ______


Basically where the ____ I'd want the customer to enter the number they wanted of each?
