A one-time set-up charge per item regardless of quantity

Hello, I've been looking for a contrib but can't find one that addresses this issue.  Any help will be greatly appreciated.

I would like to have a one-time set-up charge for an item regardless of quantity.  If I add it as an attribute for an item, the setup charge gets added for each item.

For example if you're buying 50 or 500 stress balls with your logo, there is a one time set-up charge of $30 for the imprinting. 

I can even work with something like a mandatory second item.  For example if you add stress balls to your shopping cart, it automatically adds and requires another item called setup charge.
