sudden checkout problem in high volume account

I'm experiencing a sudden difficulty with an account I use internally on a live store.  It's the account I use to log all ebay sales (so that I can keep track of inventory).  

Today, I began getting an "Internal Server Error" instead of the expected  checkout_success page after clicking on the Order Confirmation Screen.  

This only seems to be occuring in this one account, live customers do not appear to be having any problems.

I'm running a 09-Jan-03 2.2cvs version with a number of mods.  Haven't  made any changes in many months and haven't had any other problems.

Not sure if I have a table that needs repair or if it's something more serious, or if I've run into a limit on how many orders a customer can have (it's something over 400 orders for this account).

Any suggestions on what to do or even where to look would be appreciated.
