Ok, I feel like an idiot.

I caught the case sensitive thing on the @EXT but missed it on
%EXT_FILTER.

Sorry for sounding like a dumbass. Thanks for your help!

As I said: add "PDF" to %EXT_FILTER (not only to @EXT) and re-index.
That 
fixes the problem.
