Can't get past Setup/COnfig Admin Login

header.inc.php seems to get setup fine (i can view the file and everything takes) and I can log back into the Header Admin Login, but can not log into setup/config

I have read all there is to read and see no answer. I do not see where any password was set for this, but assuming it is the
'Configuration Password' this does nothing.  No matter what I enter in it and click login, it just refreshes the page, no note of incorrect password not nothing
Of course when I try to go to the index I am told no database tables have been setup and to run setup, which I cant do as I cant get past the login screen

mysql4.0
phpgroupware 9.16.012
