Many things. I was setting this up on a new server so I had:

1- Forgotten to give the database user the correct permissions
2- Forgotten to add the user to the access list
3- Trying out some changes to the source code

Ok. Definitely a problem with phpOpenTracker being called.

Did you turn on error logging and set debug level to 2 in the config file?

Did you have a look at your web server error logs?
