I realised there was one last thing that i hadn't rebooted, the underlying host that was running the virtual machines.
it probably seems kind of obvious but it didn't occur to me at the time.  i've now rebooted that and things are working again.
i'm stumped as to what could have happened to give the problems that i saw. 

anyway, i'm good, although i'm still concerned that the emergency page wasn't invoked.
