Hi here is a simple config that gives the error stated:

        HeadRequire "(Host: xxxx.mz|Host: www.xxx.mz|Host: iiiii.mz|Host:
www.iiiii.co.mz)"
        BackEnd
            Address 192.168.21.21
            Port 3128
        End
    End
End
