pyblosxom manual update

I merged Lance's changes into the manual in addition to his changes to the 
Makefile.  I vaguely remember someone having problems building the docs, 
but I don't recall who it was and I can't find the email the issue was 
mentioned in.

Anyhow, whoever it was, I'm hoping you're on this list.  Give building the 
docs a try and see if it's better/worse.

Also, I think that some things in the documentation should get split out 
and put somewhere else.  For example, the section on installing 
comments...  given that PyBlosxom doesn't come with comments anymore, 
should that be there?  Does it cause problems if the comments plugin isn't 
in sync with the manual?, ...
