formatters and entry parsers

I finally finished up most of the specification for the changes I want
to make to the transform chain that takes an entry file and passes it
through various transform callbacks which results in a body formatted
in HTML/XHTML.

One of the things this document does is codifies the behavior of the
pieces involved and the purpose behind them.  That's something that's
sort of lacking in the existing documentation.

This fixes a number of things.  For example, one of the reasons that
pystaticfile and wbgwiki don't support other markups is that
entryparsers can only parse files in the datadir.  That's a pain in
the ass--formatters should format strings, not files.

If someone wants to write examples for the specification, that'd be
really great.  If not, I'll get to it eventually.

Comments and suggestions are very welcome.  This is our chance to
discuss the specification, make sure the requirements match our needs,
and work out any other issues.
