It depends on what web-server you're using and how it's set up.  On my
server, we have the error log in /var/log/apache/error_log .  If I check
there, it tells me exactly what the errors were.

What you're seeing is that the web-server is probably running as a
different user and you need to give that user permissions to read the file
in order for it to run the plugin.

Don't know if that helps or not.  Some of the issues involved are a bit
more complicated and change from server to server.
