Just bear in mind that I'd been toying around with different approaches so
not all the examples are 'stock' Quixote forms; however the css files which
the example references might prove helpful. Also this was done eons ago - not
sure if Quixote forms have changed much since then, QP forms have a little.

Similar examples - can't recall the specific differences - probably this was
a working copy but seems to render slightly differently for a couple of
examples.

Don't mind the large fonts - I'd made everything look quite different on
purpose. 

Still like the use of the legend, fieldset and label attributes, but am not
so stuck on avoiding divs these days.
