Sure, stick with the MVC (Model-View-Controller) pattern.
A "survey application" covers a lot of potential systems. Are you
describing a system that runs the user through a few questionnaire
pages and collects the results in a database for a later statistical
analysis?
If so, such an application could be prototyped by a good WO developer
with Direct-To-Web (D2W) within a couple hours if the page design
wasn't too intricate. This could be reduced to a few minutes if the
database already existed.
