Can't indent headers anymore?

In 1.2.4, indentation of headers worked as expected.  Now, in 1.3.3,
all you get is:

  ==== Heading 4 ====

In the page output.  Is this intentional?  I checked the bug pages,
and there were no open bugs that appeared to cover this behaviour.
