Sorry, I'm a little confused.
Do you propose to use arbitrary names for separating content from 
position in the page?

I think that the various divs should be related to the type of content. 
I mean: I suppose that everyone wants a consistent visual characteristic 
for content of the same type.
In this case i think that this should have sense:

    #page

        #information-low-evidence
        #information
        #information-high-evidence
        #input-form
        #output-form

        #document

            #information-low-evidence
            #information
            #information-high-evidence
            #input-form
            #output-form

        #banner
        #gadget
