I've not encountered this particular problem, but I have experienced other strange goings on with Firefox in older CTWM versions (E.G. 3.8.2). All of them were remedied when I switched to 4.0.3.

I can use xos4-terminus in my ctwm which is built from fairly recent source, but should be fine in standard 4.0.3, too.

$ ctwm --version
ctwm 4.0.3-post
 (git:9d5cbbb9eb8d4d99772a0fc92dcdb4843d043e60)

I'm not privy to neither package maintenance nor other particularities of NetBSD (I'm on Linux) and my only association with CTWM is as a humble user, so take this for what it is, but my suggestion would be to upgrade the package to 4.0.3.
