No, mplayer doesn't automatically run full screen on under Xinerama.
I have the latest Fluxbox compiled with Xinerama and I figured out that
mplayer has a flag(--xineramascreen {0...}) that will cause it to run on
that screen. Include the -fs switch and it runs fullscreen on whatever
screen you like!
I would still like to know if there is a app setting for Fluxbox that will
cause any program to open on a particular screen.

yes, thats what it does here on my box. if you have a
xinerama-featured fluxbox and the xserver is coming up in xinerama
mode than the latest fluxbox (0.9.15.1) will make mplayer fullscreen
on one of the displays only.
if not: you are not running xinerama then or your fbox isnt compiled
with xinerama or you use an older version of fbox where things were
handled differently.
