I'm not at all sure whether this is a fluxbox or a qjackctl related bug, but I have reported this on both fluxbox and qjackctl sourceforge bug lists. I have done some testing with different window managers: qjackctl-0.2.21 works properly with KDE, Gnome, Windowmaker, Openbox and twm window managers, but has frame related problems with Fluxbox and mwm. I played around with ToggleDecor, too: with it I can momentarily force qjackctl to have frames, but if I for example hide it to toolbar and open it again, the frames are lost.
