Is it a race condition? or does it happen even if you press the keys
really slowly?

I know that there's a race condition (one that is theoretically
fixable) where the key after the prefix key can slip by ratpoison and
get sent to the focused window if you press the keys fast enough.
Dunno why you'd get t and not C-t, though. Is your control key wearing
out?
