Hi,

I can personally recommend Aspose I have used it in three web
applications now to:

Export locked forms in word format with fields (I don't know if it's
possible with pdf)
Import the returned form.
Do mail merges and export in pdf.
Create reports in doc and pdf.

I have no affiliation with Aspose.
