Are you accessing log4net in your unit tests or is it for NAnt logging?

If the latter is the case, are you perhaps running a continuous 
integration server with a build agent that sends the build log back to 
the server (and could block until the network connection times out in 
case of an intermittent connection outage)?

Otherwise, see if it happens with a (self-compiled) debug build of 
log4net, too. That would enable you to attach a debugger to the hanging 
process and see exactly where log4net is hanging.
