In the PhoneBookmenu, select the command Change Phonebook Form > New

A dialogue box pops up with all of the available folders, you can add 
any one you want into your new form.

At the top of the dialog box, there is a field to rename the form - 
right now mine defaults to Phone Form 2, but I could name it whatever I 
want.

Once you have a named form, you can select it to appear in the 
phonebook, either at the bottom of the screen or on the right. The named 
forms appear in the sub-menu under "Change Phonebook Form"
