Hello Cathy,
I bet he used to have another phonebook form that somehow got deleted.
That might be confirmed by looking at older backups, but it does not
really matter. Just assemble a new form with all the fields he wants.
The data should still be there. The form is just a view of the data in
the folders. The fields are just that entry's data in the various
assembled folders he wants in the phonebook form. I.e., fields =
folders at least in this case. --Chris
