A quick google of "custom context menu explorer" gave some interesting results. The thing is that there's a ton of small improvements like that laying about. They're just waiting for people like you to point them out, and then for somebody to hack together a solution. Just hacking together a crude solution to problems like this, and sharing it (e.g. posting on http://lsp.litestep.info), is enough. Even if it doesn't work flawlessly, if people use it someone will eventually come around with a new hack to fix those problems.
