wiki software with advanced poll?

I'm setting up a wiki to help my fellow Swedish freelance journalists
keep track of the rates Swedish newspapers, magazines, radio stations
etc are paying. In addition to the basic wiki functionality (add page,
edit page) I want to have a from based interface to a database. The
user fills out information (who was it for, how much did they pay, how
much time did you spend on it) about jobs he's done. Any wiki software
out there that does this? Anything that has a large and active
developer community gets extra points.
