In Australia at least, it is standard practice to make the author pay
for indexing. As an indexer, I am often asked to invoice the author
directly. Whether this is fair or sensible is open to question. After
all, the author doesn't have to draw their own cover art!
But the moral is to read all contracts carefully and strike out whatever
is not acceptable. My partner and I were caught out once by a 'standard
contract' - now we just go to work on them with a big black pen.
