If you are on Windows you might want to look at the Atlantis word processor. http://www.atlantiswordprocessor.com/en/ I have found that I can open a clean rtf file in Atlantis and save it as epub. Every book I have done this with has passed the validation without a single glitch. There is a 30 day free trial which is NOT crippleware, it just goes away after the 30 days. Cost to keep it is just $35 US. It doesn't have the collaboration features the more expensive word processors have but since it reads rtf and has a good styles setup this isn't a problem for me. Now, by a clean rtf file I mean one that I've saved as rtf from Word, WordPerfect, or OpenOffice. The styles translate well. I use just a few styles and don't use tables or wrapping text around a graphic. Since none of the ebook formats I am familiar with allow either this works out well. Hope this helps. Margaret I.
