A couple of responses:

Why spend $35 to see their idea of a copyright page (whether for ebooks is not mentioned) when so many examples already exist?

This is a good example of what's confusing me. The examples listed all look great. Sure wish I could copy one of these! But that would be copyright infringement, wouldn't it?

I've seen several sources that go into how to format for Kindle, or Smashwords, or whatever. However, I'm still left wondering what those requirements have to do with more general requirements, if, in fact, there are any.

Sorry to belabor this, and I'm not trying to overanalyze it. I'm just looking for someone/something that says, "X is REQUIRED, and Y and Z are optional. You are free to make up your own variation of the following text..." etc. I'm not necessarily looking for an exhaustive list of options, but at the least I'd like to know what I HAVE to do. This might sound silly, but I'm trying to avoid putting something out there and then be told later "you were supposed to do X."

This would be for an ebook for sale, in which I wish to reserve all rights and to officially state that the reader is not allowed to reproduce any part without my permission. I can come up with my own disclaimer that includes everything that I want it to say, but again I don't know if that would necessarily include everything that may be required.
