Hi, Joseph—
For EPUB 2, you need a TOC.NCX. For your list of illustrations, add a
NAVLIST below the NAVMAP. More info:

Also, in your CONTENT.OPF file, you'll have a SPINE section where you
list the files in reading order. Below the spine include a GUIDE
section. The guide is deprecated in EPUB 3, but Amazon (assuming you'll
use your EPUB as the basis for converting to MOBI) uses the guide to set
the first reading page and your in-book table of contents.

I don't know how well ereaders deal with EPUB 3. I've been waiting
before I start using it for production.
