  Okay, well, one theory is that the Dwarf is Mike's severed arm. 
Supported by the Dwarf saying "I am the arm"... and also, in that 
scene, notice that the Dwarf leans over and touches Mike on his 
stump, facing Mike, as they talk in unison. Hence, it is the 
(temporarily) reunified Mike who asks for the Garmonbozia.
  So, the question behind your question is, who the hell is the 
dwarf anyway? Is the dwarf wholly evil, partly evil, or not evil at 
all?
  There are various things in FWWM that suggest that the dwarf is at 
least partly evil, including the various shots in the David Bowie 
scene, such as "with this ring I thee wed" and the shot of him 
walking through the red curtains with Bob... and so on.
  But I don't think that the Dwarf is wholly evil, since of course 
he helped Dale at the beginning, and he has an evil Doppleganger in 
the last episode. A wholly evil being (like Bob) has no need of an 
evil Doppleganger; it would be redundant. Hence "when you see me 
again, it will not be me" is significant.
  Likewise, I'm not sure about the good/evil status of the old woman 
and grandson. Obviously they are not ordinary mortals, and they 
appear with Bob and the Dwarf in the David Bowie scene. The grandson 
says "fell a victim"... is he ordering Bob to kill someone (Laura 
Palmer)?
