without translating his questions. I hope, it doesn't lead to to much

Translating your XML to HTML *on the browser* is just implemented very 
primitively currently.

Better XSLT support is scheduled for the end of year with the releasse 
of BXE 1.1.

The only tested and certainly working solution for you, is to transform 
your XML on the server side to HTML and put in the correct bxe_xpath 
attributes like described in 
http://wiki.bitfluxeditor.org/LoadingLayout. The University of Zurich 
uses the second approach ("External XHTML"), the inline example the 
first ("Inline HTML"). The "inline" example uses an XML which doesn't 
contain any XHTML markup and works quite good.

I highly recommend you, to try to use one of those approaches

DTD? There's no DTD Support in BXE ;)
RelaxNG doesn't seem to be the problem here, as you don't come to the 
stage, where it would be loaded and validated.

Yep, your "Ansatz" is a dead end currently. Don't translate the XML on 
the browser, but do it on the server. We do it that way, the uni does it 
that way and others as well. If you really need it done on the browser 
side, you have to wait until end of the year or help out with 
implementing it ;)

Caching is always a big pain. Just Reload the Page with "Shift+Reload" 
and if that doesn't help, clear the cache... It's annoying and I don't 
know a real solution, either, but with "shift+reload" it works in most 
cases.

In Mozilla, you can have the Preferences always opened (it's not modal), 
so deleting the cache isn't "click hell", if you have the Preferences 
Box just in a corner all the time.
There's also the "Web DEveloper Tools" (an extension), which puts "Clear 
Cache" Menu entry to Mozilla/Firefox (where the Preference Dialog is 
modal...) and makes clearing the cache a 2 click issue ;)
