RSphrase is only used if you use the corresponding module. Thus please 
look at your document settings and remove the module.

Note that the R-S phrases for chemical security have been replaced by 
the H-P phrases due to international conventions. LyX offers a 
corresponding module to get them.

To answer this we need a _SMALL_ LyX example file to see where you get 
this problem and why.
