I suspect it's a matter of two different (slightly confusing) settings. In Tools > Preferences > Look & Feel > Document Handling > Windows & Work Area, the first two check boxes are "Open documents in tabs" and "Use single instance". The first one determines what happens when you use File > Open to open another document while working on an existing document in LyX. (If checked, the document opens as a new tab; if not checked, LyX starts a new instance for it.) It only affects documents opened by LyX. The second checkbox determines how documents opened outside LyX (by double-clicking an icon, or using xdg-open in a terminal, or something else) are handled. (If checked, they will open as tabs in an already running instance of LyX; if not checked, they will open in a new instance of LyX.) Do you have both checked?
