Thanks for the reply.  To answer your questions:

:echo has('clipboard')
1

:set mouse? clipboard?
  mouse=a
  clipboard=autoselect

I've now discovered that the difference is some kind of difference in the
way xterm and xfce4-terminal do the paste, not in the way vim is working.

   - Open vim in xfce4-terminal
   - Select some text
      - Middle click paste into xterm works
      - Middle click paste into a different xfce4-terminal works
   - Deselect text and exit vim in the
      - Middle-click paste into xterm works
      - Middle-click paste into xfce4-terminal does NOT work

This gives me some clues:
