Summary: comment end-tag creates element end-tag

Initial Comment:
When commenting out an element start-tag or an 
empty element by hand, the comment end-tag 
creates an element end-tag.

Work-around:
Use comment menu option, ctrl-K.

An end-tag will only be generated now if the end-tag is 
not a comment end-tag and the end-tag is not 
preceded by an end-tag.
