If you don’t mind logged-in-to-ezproxy users seeing the password, a low-tech way is to create a little html page in /ezproxy/docs/loggedin/All. Here’s my template:

All,
Currently, we are using "FormVariable" fields to automatically fill in the assigned username/password for the American Hospital Directory (ahd.com). However, they have a hidden variable named LES that changes almost daily. This causes the login to fail after users authenticate through EZProxy. We have heard from the vendor and they are not willing to change this, so we are wondering if anyone in EZProxy land has come up with an alternative solution. Currently, our stanza looks like this:
