EZProxy stanza for Tugg

I'm trying to create an EZProxy for TUGG streaming. Whats on OCLC and suggestions from searching the web are not working. Below is what we have for Tugg streaming videos. The current stanza gives a 404 error. At one point I had it working except for forward and reverse. Thoughts?
