I’ve set this up in multiple ways, but you should be able to use FormVariable: https://help.oclc.org/Library_Management/EZproxy/Configure_resources/FormVariable. Although I will point out that the example in the documentation is not correct.

Try adding this stanza, substituting your library name variable:

Title LibraryWorld OPAC
URL -Form=post libraryworld https://opac.libraryworld.com/opac/signin.php
FormVariable libraryname=YOURLIBRARYWORLDNAMEHERE
FormVariable action=signin.php

Then, use the following starting point URL, substituting your EZproxy hostname:

http://YOUREZPROXYHOSTNAME/login/libraryworld
