After posting to this list, I did receive an email from OCLC Tier 2 support:

"We are reviewing your request. We have noticed this is an issue when the set login:user is not articulated in the shibuser.txt"

I did reply to OCLC with specific cases, including the problematic of reliably pulling a value into the login: namespace when the attribute you want to use from the auth: namespace is only available via URN.

If I hear anything else from OCLC, I'll reply.
