Hello, Just want to share an update. In the end, I just added the new URL as a HJ and will switch it as the main URL once PubMed officially turn off the legacy version. The problem turned out was due to the fact that users clicked on the link to the new PubMed from the banner on the legacy PubMed. The link from the legacy PubMed, of course, does not have the "otool=msulib" string that would indicate access from our institution. Hence the missing LinkOut button for our users.
