Request copyright form

I am working on the preparations to include a package in ELPA and I noticed that the first requirement is to sign the FSF copyright assignment as the author. It is the first time that I am sending a package for consideration as author myself so, I would be thankful if you can share with me the form and some directions on what's the best way to sign it.
