It seems that TDTD author and XML luminary Tony Graham himself wrote
it:

It (the DTD) is included as part of the file tutorial.txt which is
included in the tdtd package. The canonocil home is, I believe,
   http://www.menteith.com/tdtd/
But there is also a Debian package, an RPM package, a CygWin package;
and it is pointed to by Mulberry and OASIS, at least.
