Prepend the SGML declaration (xml.dcl) before the document instance:

    nsgmls -wxml -s /usr/share/sgml/openjade/xml.dcl file.xml

In .emacs you can set:

(setq sgml-xml-declaration "/usr/share/sgml/openjade/xml.dcl")
