So I want this to be a string that is the pathname of a file

    CONTEXT-ELEMENT
          is a string, that is the name of an element type.  It can

          the elements open when the entity pointing to the current
          file is used.

Could the first line be written as "CONTEXT-ELEMENT is the name of an
element type, as a string." ?

I want be able to start my fragments with <p>, <table> or any other
tag that is permitted inside a BODY tag.  What should I set
CONTEXT-ELEMENT to?

What exceptions and short reference maps does the documentation refer
to?

    TOP-ELEMENT
          is a string that is the name of the element type of the top
          level element in the current file.  The file should contain
          one instance of this element, unless the last (lisp) element
          of sgml-parent-document is a list.  If it is a list, the top
          level of the file should follow the content model of
          top-element.

Again, an example would be helpful.

    HAS-SEEN-ELEMENT
          is a string that is the name of an element type.  This
          element is satisfied in the content model of top-element.

What is required from an element for it to be satisfied in a content
model?  What is the content model of top-element?
