[Planner] planner-erc: link following ability is greatly improved

I've tried to improve planner-erc recently, since link-visiting was
broken.  It now requires ERC 5.1 or higher.  A summary of the changes
follows.

 - When browsing an IRC link, if the connection is already open,
   don't re-open it.

 - Switch to the particular channel or user query that is specified
   in the link.  If this fails, switch to the server buffer.

 - Invoke erc-select with the correct arguments for ERC 5.1 or higher.
