Agreed, but don't ignore the start of L46B "except when declarer’s different intention is incontrovertible." If that were the case, it's hard to believe the Director would have been called, but some defenders are jerks who try to take undue advantage. Basically the Director has to decide whether declarer a) didn't notice that LHO had ruffed, or b) noticed and fully intended to overruff, but the oral instruction "came out wrong."
