Would any of our members enlighten us why it should happen and if possible, what could be done to lessen the incidence?

Bridge bidding systems have grown ever more complex over the past sixty years, making it easier for partners to forget their methods.

Furthermore, unless a partnership is very careful in designing their system, it is easy to leave gaps or create inconsistencies. For example, in Maurice Harrison-Gray’s famous article “A Short History of Texas”, after 1NT - 4C a partnership had the inconsistent agreement that 4C was both Gerber and also a South African Texas transfer to hearts.
