Ok, yes you want to avoid those, because losing your shirt year after year is bad for your career and pocketbook, generally speaking. You get older, you don't get any younger as the years go by, and it's frustrating to look back on the strategic mistakes. But tactically? That's a non-goal. Mistakes will be made, over and over again. Software is risk management, not perfection. If your code base is huge, and you keep it around for the long term, then you're going to be relying on postmortem code monkeys.
